Nameof examination: PlainMRIscan of the lumbar spine

Findings:the alignment and curvature of lumbar vertebrae are normal. Osteoporosis isfound in the lumbar spine. Anterior osteophytes are revealed at L2-5.Herniated disc of L4/5 with 4mm exceeding the left posterior margin ofthe vertebrae, obliterating the adjacent epidural fat. No other abnormalitiesare noted.

Diagnosis:Degenerative changes of lumbar spine; disc herniation of the L4/5.

检查所见：腰椎各椎体排列及曲度正常，腰椎骨质疏松，L2-5椎体前缘骨质增生，L4/5椎间盘突出，超出椎体左后缘4mm，邻近硬膜囊脂肪消失。余未见异常。

诊断意见：腰椎退行性改变，L4/5椎间盘突出。
